I experimented with this medication a few years ago and found it lacking with respect to the amount of weight that people lost.  So I quit prescribing it.  The major mechanism of action for weight loss is nausea and slowing the movement of food through the stomach, both of which can be severe and possibly permanent in some people.  People with nausea and abdominal pain may not want to exercise as much as they should in a weight loss program.  This may lead to semi-starvation as the means to losing weight and the patient losing more muscle than they should.  One report showed that in traditional weight loss programs patients lost 90% fat and 10% muscle, but with semaglutide the patients lost 60% fat and 40% muscle.

If semaglutide works for you, great.  But if the nausea progresses to vomiting, abdominal pain, or severe bloating you need to see a gastroenterologist to see if you have gastroparesis.
